Review key names

Review key names and key parsing. There is likely some inconsistencies, and bugs in parsing.

Compare keys and key combinations and see what they produce. If they don't generate the expected key, then investigate a solution.

We will also need to do this across terminal emulators and OSs.

Fix what we can, document anything that is no straightforward.

Having put together the Textual key recorder I'm now starting to test as many keys as possible, and ideally will be testing them in as many environments as possible. I'll add comments below for any unknown keys found, wrong names found, etc; broken down by environment (that is: one comment per different environment).

The major issue here is that all of the function keys, and modified home/end/page keys, all seem to simply send a unicode character rather than an escape sequence.

Keys that do work seem to work okay; but there are a lot of keys (at least with the out-of-the-box configuration) that don't make it into the terminal itself and, instead, just seem to sound the terminal bell. Also, quite a few of the modified navigation keys seem to control history buffer navigation rather than make it into the terminal and become available to the application.
