Location - 'Download Sample' should populate the headers as per location types configured

Current behavior: By downloading the 'Location' sample CSV from admin console, system generates the same CSV for all the projects which consists of headers like PHC Area, Subcenter, Village, GPS coordinates. These headers are not common for the projects and doesn't implicate the exact structure to be uploaded.

Feature Request: Location sample CSV should populate the headers based on the 'Location Type' configured in the same descending order (of location level) in which CSVs are being uploaded currently. With this change we can consider that 'Location Types' should be defined first in order to download the sample CSV and upload it back to web app with appropriate data.
