Also reference PR/issue templates from CONTRIBUTING.md rather than specifying in multiple places how to formulate these items. This means that the templates dictate the specifics of how changes should be proposed; CONTRIBUTING.md only gives high-level guidance.

This PR attempts to find procedural changes that don't touch the constitution and have been discussed in the referenced issue extensively. I will open a separate issue in order to recommence discussion on these items.
