Add primary symptom

in generate_records in synethic:

for a record, have a record['primary_sign']. (not sure if they should all be true or not)

in model.py, query:
take a record['primary_symptom']:
remove any diagnoses from potential diagnoses that do not have the primary symptom
(or just mark them)
