Having a similar issue with a single monitor. When my monitor goes to sleep and locks, after I unlock it the applications disappear. I have to go into extension settings and toggle on and off show running applications. (Just tested, your method of returning icons also works)
GNOME Shell 47.0 Dash-to-Panel 64(but I believe also existed at least on 63)

I think this is similar to my bug in #2201

Happens every time the system returns from sleep. I reset it by toggling the panel's position.

What error do you get? I get TypeError: panelManager is null.

I'm not seeing TypeError: panelManager is null in the logs when this happens

It shows in the error button in the "Extensions" app. It shows which extensions fail to load, and there's an error button which shows a popup with the reason (error message).

Maybe you have a different error?

Hmm... I'm not seeing any errors in the extensions app, it looks like everything has loaded correctly. The panel is still there and in the expected location (bottom of screen), and everything (clock, status icons, etc) is still there except for the applications.
