Replace Google Forms with alternative method of capturing customer satisfaction

Google Forms doesn't meet our needs. We should replace it with an alternative method of capturing customer feedback that:

• Allows us to pass client data about the VS Code extension to the form
• Is more professional than Google Forms
• Allows us to easily display CSAT on internal Stripe analytics dashboards
