Update value after RegisterExtension?

What happened?

in v0.10, we get response context and write or update value to Extension field that is public.
in v0.11, the extension is private, and need call RegisterExtension to set the value, and we only can get extension, but could not update extension value

What did you expect?

Any way to supdate the extension value?
