For citing, good question. I found this page: https://guides.libraries.uc.edu/citing/code, you could follow the format mentioned there, and use the github url as the web address.
For the authors, you could use my name (Chris van den Berg). @ParticularMiner has contributed a lot of the code as well (most of the code actually :)), but I don't know if he (or she) a) wants to be cited and b) how he/she wants to be cited.
