The "Create Folder" modal displays data from previously cancelled modals

Description
The create folder modal will display text that was entered on a previously cancelled modal. It will not clear this until you navigate away from the page and return.

Steps to Reproduce

1. Log in to Files
2. Click the "New Folder" button
3. Enter any text for the folder name and then cancel the modal
4. Click the "New Folder" button again
5. Observe the data from the previously cancelled modal is displayed

Expected behavior
The "Create Folder" modal should not retain any data from previously cancelled modals.
