gem_helper.rb and gem_index_cache.rb should be nuked

  • First because we want to get rid of "Helpers"
  • Next because we can do everything done here directly using CLAide::Command::PluginManager.specifications now — to know if a plugin is already installed or not, which was the whole point if those helpers if I'm not mistaken.
