I may be wrong, but it seem happened to me only when I connected the board directly to the main battery, to the balance connector, but still.

Another cause can be people holding the board in their hands "incorrectly" shortening something.

In 2 of my 3 faulty boards chips become very hot very fast, means voltage regulator does not work. Same happens when powered with 5v via "HC-12" connectors.
The third board was in a big spectacular crash and got some parts ripped off, so not relevant for this issue :)

can you confirm if local +5V are still available on the board after the problem occurred for the first time?

Did not happen for me with HW v2.6 at least.
