What/Why

Implement the view of the evaluation form without submitting the draft or completed scores to the backend. The evaluator can interact with the form but it will not make any backend requests.

Acceptance Criteria

• The evaluation form is rendered for the evaluator's view
• Evaluator can enter scores for numeric, binary and rating scale criteria
• Evaluator can enter comments below each criteria score
• Evaluator can enter additional comments below the total score summation of the form

Disregard the following if this work does not include any UI changes

• WAVE screenshot
• Automated a11y tests (axe-rspec specs)
• Keyboard accessibility (tab navigation, form interactions)
• Responsiveness at desktop and mobile
