Add chapter confirmation screen

After students have selected a chapter, they should view a chapter selection confirmation screen. This screen pulls in the following fields:

• ChA primary contact
• Chapter name
• ChA email address
• Program link

Should this page should have "go back" and save buttons? We also need to actually save the chapter assignment, which should probably come after the back-end refactor, #5024.

Good question - I had assumed that the save happened when they selected the chapter, but it's probably good to explicitly confirm that is the chapter the person is selecting.
