  • why does this happen? Conflict aversion in people with merge rights, unclear authority to overrule objections, backlog of uncontroversial work
  • how can we fix it? Clearly define what objections can be overruled and why. Differentiate between fixable mistakes, and unfixable mistakes to triage cost of getting it wrong
