Create a public form page which will be opened and filled

create the structure of form using the form structure of a particular form code.
add field type validation as per the form structure. try to use zod for validation.
Add a feature for users to delete form data.
Add an option to create an ID on the platform for auto-fill.
follow the design guide and use shadcn-ui
