NodeDiscovery threads have non-unique name #626

There is no thread name format provided to the ThreadFactoryBuilder, so they get the hard-to-understand name pool-5-thread-1.

Adding a .setNameFormat("AstyanaxNodeDiscovery-%d") should fix it.
