Merge NuGet.VisualStudio.Implementation into NuGet.Tools.

The primary motivation is performance benefit.

We don't really have much to gain by having 2 separate assemblies. You could argue some separation of concerns, but that doesn't mean it needs to be a completely separate assembly considering we don't ship it as a package.
