Included Source Codes: do not show counts of source concepts derived from the STCM and having concept_id = 0

Currently Included Source Codes also picks up concepts that don't exists in concept table, but exists in source_to_concept_map with source_concept_id = 0.
Achilles_result_concept_count table used to derive counts of concept has 1 row for concept_id = 0 with sum of counts of all different source concepts with source_concept_id = 0. This result might be helpful if user wants to see the number of custom source concept used, but it's misinterpreted by Included Source Codes statistics, which shows each source concept has that summary count:

To fix this I would recommend to show NA instead of a number for a concepts with concept_id = 0
