User Importer issues - Possibly related to Version 4.0.835.0?

Describe the bug
Ombi V4 just marked everyone (users) as a jellyfin user, it was all setup as Emby. I re-imported everyone. Anyone with a Emby Connect account was loaded as a jellyfin user, the rest corrected as a Emby user.

When I noticed this issue it had also changed some users "username" to alias with the user name blank. I was not able to correct it so I had to blow away those users. It came up with the error the user name was incorrect when trying to fix the entry. Re-Importing fixed this issue.

It also had some graphical issues when running the importer. (Empty white box in the left hand top corner)

Last note, when importing Emby users, the user name changes to the Emby-Connect username, not the emby server username

Steps to reproduce the behavior:

1. Go to '...'
2. Click on '....'
3. Scroll down to '....'
4. See error

Expected behavior
Emby users to be loaded as Emby users

Fixed in the latest version, the UI was just showing the wrong label, there's no data corruption
