Error page prompts user to submit problems to a website that is no longer functional #31

When the user receives a fatal error they are directed to a form which prompts the user to enter information about what they were doing and includes phpinfo from their server. The form posts to "http://www.phalanx.nl/pperror.php" which no longer exists.

If we can get the code for that page we can recreate it, otherwise we need to recreate it from scratch based on the input fields. It should be hosted on something.projectpier.org.

The easiest way to test this is to turn off your local mysql service and browse to your local pp installation.

@phpfreak was this script on a server of yours? Or do you know who could have a spare copy of it lying around?
