Standardize CouplingMap.graph node data as physical qubit index #9851

Following from discussion in #9710 (comment)_ , we should:

  • standardize on using the graph node mapping as the canonical location of the physical qubit index
  • Investigate relaxing contiguous zero-index restrictions on CouplingMap (sparse distance_matrix, etc.)
