Some UI UX Glitch to be fixed

If you go on add liquidity page on any pool and want to pool asymmetrically, you uncheck the pool equally button. Once you click to uncheck the Whole top bar disappears along with the pool equally button and you can not check or uncheck the button again unless you go back and start all over again

Also, Pool Equaly is misspelled, it Equally

not reproducible after patch 2.6.7
