We'll, I've been trying to create a Diabetes prediction system using Django on PyCharm. - [ ] I've done almost everything, I get this error

I don't know why am I getting this, I've checked the spaces, the casing everything is correct the axis as well. I'm kind of new to machine learning and coding like this. Please if someone can help me I'll appreciate it. I will provide all info needed to solve this
