Re-order and re-write the questions on the "money you spent" screen - Report of Dedicated Account

The 2nd question “Balance in dedicated account as your reported on a prior report” should be the first question asked since the instructions say to “start by telling us how much money was in the account as of your last report to SSA”

INCLUDE A HELP WINDOW FOR EACH OF THESE QUESTIONS TO CLARIFY TO THE USER WHAT IS BEING ASKED + RE-WRITE THE QUESTIONS TO MAKE IT EASIER TO UNDERSTAND

There are instructions for each of the questions asked on this screen that show up under the completed form. We should be including the info in these instructions in a “?” or “Help” window while the user is on this screen
