Potentially separate the type of discharge and patient status at discharge

This is the current discharge modal in BHIMA:

On discussion with our Vanga colleagues, they propose breaking this into two fields:

1. Type of Discharge (including Regular Discharge, patient fleeing, etc)
2. Status at discharge (death, cured, improved, deteriorated, etc).

For reference, at Vanga, the four types of "etat a la sortie" are "GTDE" - healed, transferred, death, and escape.
