Add description of suff_report and insuff_report JSON structures

The json_record for both the suff_report and insuff_report tables are a JSON array of values. Nowhere is there an explicit description of what each column in those arrays represent.

The columns are created in the ge-report/create_hu_report_map.py script. At the very least there should be a description somewhere in the HTML of what these columns represent.

The columns are:

1 allele_freq
2 autoscore
3 clinical
4 evidence
5 name
6 suff_eval
7 variant_impact
8 zygosity
9 expect_effect
