Case index add button

Current implementation of + New case button doesn't look super intuitive and mostly users have feeling of Do search button because of the position.

I've made couple of version which looks a bit better.

I think the best version is 4. so I've implemented it in #570 PR

Hey @davo11122 - thanks for playing with this. I feel like centering the button creates a somewhat odd new element in layout. Any chance I can sell you on #3?
