Problems during set-up: to be included in the documentation?

What I found during the setup were two things that are not crystal clear in the documentation:

• The Mautic form needs to be a campaign form, not a standalone form
• The labels need to be small-caps, words with capitals don't give empty fields

Please let me know whether I'm wrong, or could you otherwise include it in the documentation? I spend an hour figuring this out.

Note to all plugin users: YOU HAVE TO USE A CAMPAIGN FORM NOT A STANDALONE FORM - otherwise it won't work properly.
