Tech Preview UI displaying "Create question" survey button greyed out

Bug Summary

When creating a Survey within a template on the Tech Preview UI the first question is created without issues. When attempting to add a secondary question to the survey the button is greyed out. If i switch to the old interface i can add additional survey questions with no issues.

Steps to reproduce

1. Create new ansible template
2. Create survey and add initial question
3. Attempt to add another question and the "Create question" button is greyed out

Working in old UI
