Also need to modify CosmosDbOptionExtension.ExtensionInfo.GetServiceProviderHashCode()

- Change CosmosDbOptionExtension.ExtensionInfo.GetServiceProviderHashCode() to include new ConnectionMode option;
- Fix typo in comment.

@AndriySvyryd, I added the connection mode to the CosmosDbOptionExtension.ExtensionInfo.GetServiceProviderHashCode() method.
