No widget shown in latest release

@woloski and @rolodato have both updated to the latest version of the Plugin and the Widget is not shown anymore. It's not that it's hidden it just isn't in the HTML.

Seems that the issue is that the CDN is wrong.

Checking on http://auth0-wp.azurewebsites.net/ it is pointing to http://cdn.auth0.com/js/lock-6.min.js instead of cdn.auth0.com/w2/auth0-widget-5.min.js.

This endpoint is changed on the working branch (a0-widget) but not on master. I'm not sure why this has changed.
