Namerequest UI - previous value not clearing in continuation in flow

Describe the bug in current situation
When switching from continuation in of a Limited Company (and choosing numbered), if you then switch to Cooperative Association, the 'Go to Corporate Online to Register' button is not clearing from the screen. (This also happens if you start with a Benefit Company or a Community Contribution Company)

Steps to Reproduce
Steps to reproduce the behavior:

1. Select Continuation In as the action in Name Request
2. Select Limited Company as the type of business
3. Select Numbered Company
4. Change business type to Cooperative Association
5. See error

Expected behavior
GIVEN I choose the wrong business type when selecting my name option
WHEN I change to the correct business type
THEN Any items on the screen that refer to the previous business type are removed.

The bug is that two action buttons are displayed. As this is a Continuation In, the correct action button here is "Check this Name".

The root cause is that some variable(s) did not get reset when changing the flow (action).
