Edit NR - User can input the third choice without input the second choice

Describe the bug in current situation
For NR with no designation types, when user edit NR, they are able to input the third choice without input the second choice and save.

Checked the number of NRs that has third option but does not have second option is 3198, about 0.31%

Steps to Reproduce
Steps to reproduce the behavior:

1. Go to Name request and request a new name that has no designation types. Input first choice only.
2. Click on submit and follow the flow to finish name request
3. Go to edit page after NR created
4. Input third name choice, leave second choice blank

Actual/ observed behavior/ results
NR was updated without 2nd choice.

Expected behavior
User should not be able to input third choice without second choice, just like editing other type of NRs.

I feel like even thought the impact is minimal it's important to have the names entered in sequential order, because the examiner is likely to think that there is a "missing" second choice which will lead to unnecessary back and forth with client.

I tested in Test environment and was not able to create a third choice without inputting a second choice first

I was only able to proceed with the edits when i filled in the second choice before the third
