Today, when a routing slip is created, we don't capture information about the person or business that sent us the money. That information is only filled in when a refund is requested.

The goal of this ticket is to capture the name of party that paid for the routing slip at the time a routing slip is created.

Current state - missing person or business name:

Refund Request Form Unresponsive on Dev Environment

Environment:

URL: https://pay-web-dev.web.app/
Component: Routing Slip -> Edit Status -> Refund Request
Observed Behavior: The form does not respond upon clicking "Done" after all required fields are filled.
Expected Behavior: The form should process the input and complete the refund request.
Steps to Reproduce:

Navigate to https://pay-web-dev.web.app/.
Select an active routing slip.
Click Edit Status and choose Refund Request.
Fill in all required fields.
Click Done.

Notice, the form has no response
