Entry file not checked for modifications #8

Modifying the entry file does not cause a rebuild.

Linux Mint 18.3, Node v8.10.0, Express 4.15.5, TypeScript 2.7.2

Changing resolved.resolved to [{ path: ts }].concat(resolved.resolved) at middleware.js:146 should fix the problem.
