It looks like the chief fields we'd be interested in are "Applicant", "Project Description", and "Owner" - some or all of which we may want to merge with the existing "Project Name" and "Address" fields in the existing template. This will require some careful thought (do we have any idea what these fields mean??) and you may want to work with whoever picks up #19 to create, say, collapsible table rows that show more information.
