Contact diary: export entries in SORMAS-compatible format

Current Implementation

Currently the export of CWA's contact diary looks like this:

Suggested Enhancement

Offer two export-formats:

1. MD-TXT (current implementation)
2. SORMAS-compatible CSV (enhancement)

The popular contact-diary app Coronika has these two export options, a sample CSV-file I created can be downloaded here. The table structure looks like this:

The user should be able to choose the format when exporting their contact diary.

Expected Benefits

Local health authorities are increasingly encouraged to use SORMAS as their main contact surveillance software. Giving users the possibility to export their contact diary in a format which can directly be imported into SORMAS without the need to convert or manually enter the data could speed up the whole process.
