I know this is already on the mental list, but a few observations:

  1. I very occasionally notice the full window flicker of a slow file replacement.
  2. replace-buffer-contents might be useful? I'm not sure what 'accessible' means here https://www.gnu.org/software/emacs/manual/html_node/elisp/Replacing.html#index-replace_002dbuffer_002dcontents.
  3. The other common thing seems to be using RCS patches e.g. https://github.com/doomemacs/doomemacs/blob/3b789752a30419535b328c605957b6b43a9d47fa/modules/editor/format/autoload/format.el#L38. I couldn't find anything built-in or within ediff that looked perfect for what we want, but I didn't look too closely.
