When a custom URL (and model) is enabled these need to be shown in the dropdown.

Actually, it is shown, but under OpenAI provider

I've introduced a "CustomOpenAI" provider, this way we can remove "Exo" and "JLama" and have a dedicated drop down for that. Also the "OpenAI" drop down is only shown when you provide an API key which makes it confusing.
