Describe the Bug

Regarding a couple of raised issues, such as:

1. [BUG]: Migrations are not generated for new Indexes #3873
2. [FEATURE]: order indexes for columns #3861
3. [BUG]:Composite primary key not added to postgres schema when using array return in pgTable #3596

In order to reproduce the issue, grab and code snippet from the issues given above and attempt to generate migrations.

The solution I've found and suggested works but I've been wondering if that was the intention and the docs were not updated or is it a postgres/other drivers issues related issue(s)?
