Inline code in page title not rendered as expected.

Expected behavior

Page titles with Markdown inline code syntax should either be formatted as inline code or plain text. Either way the ` should be removed.

Steps to reproduce

This seems to be a widespread issue, but here's an example:

1. Build the elasticsearch.md doc set
2. Go to http://localhost:5000/zip-windows.html

I think this will be fixed by #193

Doing the following works as expected:

I can confirm this was fixed by #193.
