Modal does not receive focus

When modals show up, they don't get focus by default, so none of the keyboard controls work. You either have to click inside the modal somewhere to focus it, or tab into it.

The confirmation modal at least has a workaround where you can use defaultFocusedButton, but for normal modals there's no such option. If there's something I'm missing, then perhaps this is just a docs issue.

I cannot reproduce this issue any longer, it was likely addressed by ongoing focus trap work (see also #3889)
