Problem
Currently in Time to Visualize, there are a few small discrepancies in the breadcrumbs between the Lens and Visualize editors. These breadcrumbs should be unified to present a consistent UX before the Time to Visualize project can be enabled by default.

Since the breadcrumbs on Lens have had design input, the visualize editor's breadcrumbs should be updated to match.

The two small changes in the visualize app are as follows:

Clickable Originating App breadcrumb

This should work similarly to the cancel button, which redirects to the originating app (dashboard or canvas so far), and shows a prompt if there are unsaved changes.

Removal of the 'visualize' breadcrumb when editing or creating in by value mode

This further separates the by value mode from the visualize library. When editing or creating a visualization directly from the dashboard which doesn't exist in the library the only breadcrumb should be the originatingApp

Lens Create From Dashboard @ryankeairns This one should probably be updated to read 'dashboard / create visualization' correct?
