new instructor creating should fetch name from employe record

when creating new instructor, the name of the instructor is not fetched from the employee record.
it should be fetched from the employee record, not entering new name

This is for flexibility reasons.
If the user does not want to link the instructor to any employee, they can leave that field blank.
