'Same-Origin' 401 error on iOS Safari

Hi, this used to work great for me thank you so much for your work on it. In the last few months though, possible since the api v4 integration, I get a 'Same-Origin' 401 error response on iOS. Its something to do with the iframe and how it loads. Any thoughts why this is happening?

Full error log:

[Error] Failed to load resource: the server responded with a status of 401 (HTTP/2.0 401) (formResponse, line 0)

[Error] Refused to display 'https://docs.google.com/a/createdesign.tech/forms/d/e/1FAIpQLSf2bFBcN80AQuemUf4pagoEhii7mdONge68u6BejuLoxkz3_w/formResponse?&submit=Submit' in a frame because it set 'X-Frame-Options' to 'SAMEORIGIN'.

If the iframe source and the site source are not the same then you will receive this error.
