I think idena needs solutions early in the game, otherwise the publicity for upcoming idena oracles could suffer?
In such a example case like above, participants of the vote should be able to call in a bigger jury to jugde and reconsider. In case that jury comes to a different result and reports the unreported flip, the costs (maybe accounted for in __ x current idena hourly mining reward) should be recovered from the stake of every participant of the former jury (as well as the compensation for the victim of wrong decisions). If one of those jury mambers has not enough stake, its identity should be set to suspended, zombie or worst case even get killed.
People creating consensus have to take it more serious, or we all suffer!
Another straight forward action: forbid people to skip the reports-section after long session. People who run two identitys at once likely skip that section anyway allways. They shouldnt be allowed to?

We need mechanisms to challenge obviously wrong consensus. A user at telegram shared that example of a flip that wasn't reported but should have been: https://scan.idena.io/flip/bafkreid2v6qc3aeuvb623iaxmwsocg3nssf3yxls7ko2ki6r5vwe6t77by
