ipynb reader - include fallback for output

Currently the ipynb reader does not include the text/plain version of output if a richer version is present. However, that means that there is no fallback for formats for which a richer version isn't defined. It is probably better to include the fallback as a specially marked code block, and add some logic somewhere to omit this when it's not needed.
