This is part of "Documentation clean up" that had finished five months ago. I am supposed to stop accepting responses and remove link to the form and compile the results, but I had forgotten this all together until today. I shall compile the results and update here by early September 2016.

The availability
The user feedback form is created using Google Forms. It is the second revision of form that has been published in early March 2015. The form is accepting responses until end of August 2016, this month. The form can be accessed from GitHub Wiki sidebar or at this direct link (now closed).

Not sure if i broke something in the image, but it does not work in QUEM (does not boot).
Entry dated 2016/01/20:
Install qemu-system on Debian. Keyboard keys don't work after successful boot into command prompt (likely QEMU issue).
Entry dated 2016/08/26:
Haven't throughly tested but surprise; It is possible for old stable to rebuild ISO image of 16.04 release!
Most unique experience of remastering:
one participant had installed Customizer on Ubuntu 64-bit host system and successfully remastered Ubuntu Mini Remix 32-bit ISO image, with core features mostly work "perfect"

The feedback form does not record accurate Ubuntu release information, because this will make the form complicated; The first version of feedback form had attempted this and was tedious to fill
