Redeem bug - using same testBTC address for redeem decrease chances

Description:

At this point it's hard to report this bug very clearly, as there are lots of transactions made, but here's quite valuable observation:
90% of redeem transaactions made to same testBTC address doesn't go through fully (see screenshot below). Most likely only first redeem will send 1 test BTC back, while next times it will just burn my tBTC and send nothing.
While if I use different address every time, most likely that I will get 1 testBTC back to the address. What could cause such weird issue? I haven't seen anything similar reported yet.

Expected behavior:

Receiving 1 testBTC regardless whether address was previously used for Dapp or not
