デフォルトのurnidがUUIDではなく空になっている #637

Ref #636
「ReVIEW::Configure側で上書き」によってurnidがUUIDではなくnilとなるため、
config.ymlで明示的な設定をしていない場合に壊れたEPUBが作られてしまう。

とりあえず #636 をマージしました。

ほかにもconflictしてしまっている設定があるかもしれないので、あとで見直していきます。
